New patients are advised to arrive at the practice 10 minutes prior to the appointment time to complete the new patient registration form. Alternatively the form can be downloaded here and completed prior to the visit. Remember to bring your Medicare Card if you have one.
If you are coming off Ashford Avenue, we are located behind the Post Office. Be aware there are 2 medical practice in close proximity. We are NOT Milperra Medical Centre. Our surgery has grey walls and you should look out for our blue and green logo.

Appointments
Appointments can be made by calling our friendly receptionist on 02 9774 3388, online on our website or via the HotDoc App on your smartphone.
Please note that emergencies are given priority and our staff will attempt to contact you if there is any unforeseen delay in your appointment.
A standard appointment is 10-15 minutes and our doctors can usually deal with 1 - 2 issues during this period. If there are more than 2 issues we recommended booking a long consultation. Please advise our reception when you book your appointment.

fees & billing arrangements
Medicare Card Holders
Patients who have a valid Medicare card will be bulk billed. Please bring your Medicare card and present it at reception.
Procedures
Our practice charges a small fee for procedures. This is to cover cost of the equipment required to perform the procedure. Such procedures include excision of skin cancers, Implanon insertion and laceration repairs. The fee will be discussed and consented in a consult prior to the procedure being carried out.
WorkCover
Patients who present to the practice for WorkCover related services are charged AMA WorkCover rates. We ask patients to settle the account on completion of their initial appointment. An invoice will then be issued allowing you to make a claim with the insurance company involved. Should further WorkCover appointments be required, the patient will need to obtain a claim number from their employer/ insurance company.
Non Medicare Card Holders
Non Medicare Card holders including those on Overseas Health Cover (OSHC) will be asked to settle the account on completion of their appointment. Fees are based on the length and complexity of the visit and services provided. This will range between $60 - $150. Please enquire at reception about current fees.

We offer services which include completion of workplace medicals and insurance applications. These services will be privately billed and current fees are available at reception. The service fee will be required to be paid upon completion of the appointment.

Recalls
Our practice aims to provide preventative care to its patients. Part of this will be seeking your permission to be included on our reminder and recall system.
From time to time, we will contact you with regards to preventative care such as immunisations, pap smears, faecal occult tests. If you do not wish to be part of this system please advise your doctor or receptionist.

Results are not given over the phone. Appointments are requested for results to be given with adequate follow up.
after hours coverage
When the practice is closed, for non urgent medical care you can call National Home Doctor on 13 7425. If it is an emergency, please present to your nearest Emergency Department or dial 000.
Telephone access
Our doctors may be contacted during normal opening hours. If the doctor is unable to take your call, you may leave a message and our reception staff will advise you when it will be likely the doctor can return your call.

Email correspondence is managed by non clinical staff. Hence, the practice will not be responsible for any health information communicated via email. Unsecured electronic mail is at risk of interception by third parties, non- receipt or delayed receipt of the message.
