All our doctors have an interest in managing chronic disease. We are able to provide chronic disease management for common chronic diseases, including the following, but not limited to: - diabetes - hypertension - kidney disease - heart failure - asthma - COPD/ emphysema - breast cancer - inflammatory bowel disease |
Our clinical team strive to formulate plans to help you manage and understand your condition. We work in conjunction with your hospital based specialists as well as allied health providers, including physiotherapist, dietitians and podiatrists to achieve your goals and best manage your condition.
If you have a chronic condition, you may also be suitable for a GP management plan, othewise commonly known as a "care plan." If you have a chronic disease meeting certain eligibilty criteria, you may be able to access a "care plan" which will help you access up to 5 subsidised visits to an allied health professional per calender year.
In our current world of medicine, treatment for chronic diseases continue to change at a rapid pace. As a teaching medical practice we strive to keep up to date with evidence based medicine to provide the best care for our patients.
