If you have a chronic illness for more than 6 months, you may be suitable for a care plan. There are 2 components to a care plan
1. GP Management Plan (GPMP)
2. Team Care Arragenements (TCA)
A GP Management Plan is a document which outlines the medical issues you may have and outlines goals and directives. For example, if you have asthma, the goal is to make sure you are not breathless and we can acheive this by ensuring you take your puffers. This plan is given to the patient as well as any other specialists or allied health that is under your care.
If a person has more than 2 other specialists or providers, a Team Care Arrangement can be organised. This is similar to GPMP as it conveys the treatment goals to the other providers. For example, we can instruct the same patient with asthma to see their lung specialists to have annual check up and a spirometry. The benefit of having a GPMP and a TCA is that it allows you to have 5 subsisised visits to see an allied health professional. This includes
a. physiotherapy
b. occupational therapist
c. dietitian
d. podiatrist
e. exercise physiologist
Please note not everyone can have a care plan. As Medicare hav strict eligibility criteria, only your doctor can approve whether you are suitable for a care plan. For example, if you sprained your ankle last month, you cannot have a care plan to see a physiotherapist.
